Trump picked General Mattis as his Secretary of Defense purely because he had the nickname "Mad Dog", then got upset when Mattis wasn't a psychopathic warhawk who'd do anything Trump wanted.

SO usually, when you or some one describes some one as a puppy killer, its done in hyperbole, often in a negative framing: "Its not like she's a puppy-killer". Basically, she like.. humblebragged? about taking a puppy down to a gravel pit and killing it in her auto-biography.
She killed her puppy, who she was training to hunt fowl, after it killed a chicken or two at a ranch.
