I see a lower intestine, stinky and loaded with danger.
That's the best I could see at first too.
But then imagine my surprise when i looked again and suddenly saw it's a woman with her mouth & fist wrapped around something, most likely an erect penis, But the penis isn't visible in the painting because her mouth and fist are covering it.
move the picture further away, blink a couple times.
