I always just assumed it's another visual for porn. There is only so much you can look at, so it helps add more for the viewer to think about. It looks hot, but in reality, I would imagine it's rarely done. I have never done it either.

Vagina may not always be wet enough. Not all women produce enough lube, even when much aroused. It essentially boils down to physiology.
